Apprenticeship with Uncertainty Coaching Agreement

Coaching  is a collaborative process with an ongoing relationship between you, the Client, and me, the Coach. The experience supports you in establishing new behaviors, beliefs, and ways of being. This process in intended to help you transform your relationship with uncertainty and bring you more calm, trust, and groundedness.

The specific coaching agenda is implemented in partnership between you and me. My role is to guide and support you in what you need and want to achieve. Specific outcomes that emerge depend on you, and how you engage in the coaching process. I will offer you prompts and invitations for engaging with the growth process between sessions. Coaching works best when we both engage fully in the coaching experience. We agree to bring our whole selves to the process, to be honest and authentic.

Coaching is not therapy or counseling, and is not a replacement for therapy. As your coach, I agree to keep all conversations and information with the you private and confidential, as allowable by law. No personal information will be shared with anyone without your express permission.

Coaching sessions will occur virtually via video conferencing. I will schedule and initiate meetings/calls. We agree to start and finish appointments on time. If you are more than 15 minutes late to an appointment, I will assume that the appointment is canceled and you will be responsible for the full fee. If you need to  cancel or reschedule an appointment due to illness or other circumstance, please do so at least 24 hours in advance. I also reserve the right to reschedule an appointment.

Once you submit this form, I will contact you to schedule our first coaching session. We will meet monthly at times that we both agree upon.

I understand that Apprenticeship with Uncertainty is a yearlong coaching container. I agree to either pay in full, or make 12 monthly payments for this coaching experience.
